'Doctor Shin' Jung Yi-chan carried out a second brain-change surgery that swapped the brain of Song Ji In, which was inside Baek Seora's body, with Cheon Young-min's, delivering a shock.
On the 29th, episode 6 of TV CHOSUN's weekend miniseries 'Doctor Shin' (written by Phoebe (Im Seong-han), directed by Lee Seung-hoon) showed Shin Ju-shin (played by Jung Yi-chan) proposing a brain-change surgery to Kim Jin-ju (played by Cheon Young-min) after reaching the height of conflict with Momo (played by Baek Seora), who possessed the brain of his mother-in-law Hyun Ran-hee (played by Song Ji In).
Shin Ju-shin felt perplexed as Momo (with Hyun Ran-hee's brain), who began living with him, kept meddling and even provoked him. Momo (Hyun Ran-hee's brain), ignoring Shin Ju-shin's remark that it was troublesome, insisted she would prepare the birthday table herself, admonished Shin Ju-shin's polite speech, and even made him uncomfortable by mentioning his closest aide Ha Yong-jung (played by An Woo Yeon), saying, "Men have no secrets, are you going to tell Ha Yong-jung? Betrayal usually comes from the closest one."
Meanwhile, Kim Jin-ju, after being told by Geum Ba-ra (played by Joo Se-bin) about an independent living support center, met Kim Gwang-cheol (played by Cha Gwang-su), and when she heard that he was looking for Kim Jin-ju, not Geum Ba-ra, she was filled with anticipation at the prospect of meeting the father she had dreamed of her whole life. When Kim Gwang-cheol, upon meeting her, confidently said, "I'm totally your mom in my 20s," Kim Jin-ju poured out her resentment at being abandoned while also feeling overwhelmed.
But when Kim Jin-ju followed Kim Gwang-cheol for a meal, she was shocked to see an electronic anklet on his ankle. Furthermore, when Kim Gwang-cheol confessed to having a criminal record including imprisonment and brazenly insisted on living with Kim Jin-ju in her one-room apartment, Kim Jin-ju returned home after breaking up with him, sang a self-mocking hymn, and expressed a sense of emptiness and despair.
Afterward, Kim Gwang-cheol went to James's (played by Jeon No-min) Rim Company, ranted that he was Jin-ju's biological father and that she had not contacted him for ten days, and demanded the home address; despite James saying he had quit, Kim Gwang-cheol and one of James's male employees eventually broke down the front door of Kim Jin-ju's home and entered. On the bed at Kim Jin-ju's home, there was a suicide note implying death that read, "I'm so tired and my last hope is gone."
Upon hearing the news, Geum Ba-ra shook her head in disbelief at Kim Jin-ju's suicide note line "I'll find peace in the water," but she could not hide her astonishment when the police confirmed that Kim Jin-ju's cellphone was turned off at Yeongdeung Port and that CCTV at Yeongdeung Port captured Kim Jin-ju's last appearance.
At that moment, as Shin Ju-shin put on surgical clothes, Park Su (played by Park Joo-yong) carried the unconscious Momo (with Hyun Ran-hee's brain) and laid her on the operating bed; with Kim Jin-ju on the left and Momo (with Hyun Ran-hee's brain) on the right, Shin Ju-shin began the brain surgery, heightening the tension.
A flashback then showed Shin Ju-shin having dinner alone with Kim Jin-ju. Shin Ju-shin said to the unhappy Kim Jin-ju, "Don't you ever think of lying down on the operating table? If you do, you can change your life," and with a meaningful expression added, "life plastic surgery? Don't you want to live like Momo?" He also warned her to keep the secret until he died and said, "Right now Momo is only Momo on the outside. Her mind is a complete mess. The proposal came from Momo's mother, but either way the surgery was done. It's a success," and added, "We decided to change the brain and part ways naturally, but she had other ideas? No matter how much she looks like Momo on the outside, her insides are the prospective mother-in-law — is that possible?" which shocked Kim Jin-ju. Shin Ju-shin then made Kim Jin-ju's heart flutter by saying, "This isn't coercion. But I want it to be you," and captivated her with, "What do you think? Don't you want to experience being just the man Shin Ju-shin? For some time now I feel Kim Jin-ju is slowly settling in my heart."
Eventually Kim Jin-ju, asking, "What happens to me if it fails?", imagined a hellish life with her biological father Kim Gwang-cheol, and showed signs of wavering at Shin Ju-shin's enticement to become Momo and have everything, and at Shin Ju-shin's firm assurance not to worry about Hyun Ran-hee. After dinner, back at home, Kim Jin-ju faced Geum Ba-ra's photo on the wall and muttered with a crazed smile, "Geum Ba-ra showing off. Let's see what happens to this Kim Jin-ju. I'll make this Kim Jin-ju Momo.." She then swept items into a trash bag to tidy up. Later, at the beach, Kim Jin-ju lamented, 'Kim Jin-ju lived a sad life,' took a final selfie, threw her cellphone into the sea, and got into Park Su's waiting car to leave.
At the end of episode 6, while Shin Ju-shin was absorbed in brain surgery wearing goggles, scenes alternated with Hyun Ran-hee's face mirrored on Momo, who lay face down on the operating table, and Kim Jin-ju waking up as Momo on the recovery bed, wide-eyed in terror — a "pupil-shaking ending." It raised tension over whether Shin Ju-shin's brain-change surgery was signaling success and the next waves of turmoil about to hit.
