On the 22nd broadcast of TV CHOSUN's weekend miniseries '닥터신' (written by Phoebe (Im Seong-han), directed by Lee Seung-hoon), episode 4 unfolded a twist in which Hyeon Ran-hee (played by Song Ji In), who came to live in the form of top actress daughter Momo (played by Baek Seora), Shin Ju-shin (played by Jeong Yi-chan), who was bewildered and could not accept it, and Ha Yong-jung (played by An Woo Yeon), who liked Momo, displayed different desires, true colors and madness intertwined.
Momo (Hyeon Ran-hee's brain) broke into a heartbreaking cry in front of the police and paramedics who responded to her report, then burst into the room where Hyeon Ran-hee (Momo's brain) had been, calling for her mother and sobbing, which quelled everyone's suspicions. Shin Ju-shin, who arrived late at Momo's house, felt heartache as the ambulance passed and thought of Momo. As Shin Ju-shin prepared to cover the body of Hyeon Ran-hee (Momo's brain) brought to the hospital with a sheet, he alternately looked at the truly deceased Momo and the outwardly Momo (Hyeon Ran-hee's brain) who was sobbing violently beside her and revealed his sorrow. Before the casket was closed, Shin Ju-shin looked at Hyeon Ran-hee (Momo's brain) and, choking up, kept repeating, "Momo, did I do the right thing? Do I have to send you off like this?" which evoked pity.
Later at the crematorium Shin Ju-shin tried to restrain his emotions, but Momo (Hyeon Ran-hee's brain) shed endless tears and bade farewell to Hyeon Ran-hee's body that Momo's soul had entered. After the funeral, when only Shin Ju-shin and outwardly Momo (Hyeon Ran-hee's brain) remained, outwardly Momo comforted the confused Shin Ju-shin, saying, "When you look in the mirror you don't feel like Momo is dead. The one who's dead is Hyeon Ran-hee. Just accept what you see." But Shin Ju-shin averted his gaze from Momo, who looked like Momo on the outside but was Hyeon Ran-hee in essence. Suddenly, when Momo (Hyeon Ran-hee's brain) approached saying she would loosen his tie, Shin Ju-shin was flustered and, feeling uncontrollable emotions, returned home conflicted with confusion and anguish.
Meanwhile, Ha Yong-jung recalled the past when he fell for Momo at first sight. He had been captivated by Momo as she approached him dancing and offered skinship while he was playing the double bass at the Nuhae Foundation orphanage's "sponsorship appreciation night." Ha Yong-jung asked his older brother James (played by Jeon No-min) to introduce him to Momo, but when he heard of Shin Ju-shin and Momo's engagement he was devastated. Later, invited to the engagement ceremony at Shin Ju-shin's villa, Ha Yong-jung tried to compose himself and smiled, though inside he was again struck by Momo. When Momo said she wanted to dance a waltz with Ha Yong-jung, he imagined holding her hand and dancing a waltz, revealing an overwhelming, uncontrollable longing.
Then James insisted that he would deliver side dishes to Momo's house for Geum Ba-ra (played by Joo Se bin) who had held Momo's funeral, and Ha Yong-jung insisted on going himself. Bringing the side dishes, he comforted Momo, saying, "We're all in the same situation. If you have no parents, you're an orphan," which cheered her. Momo (Hyeon Ran-hee's brain) broke into tears, but when Ha Yong-jung sympathetically handed her a napkin, she inwardly thought, "Shouldn't I hug him?" revealing a desire-driven twist that shocked. Moreover, after Ha Yong-jung suggested, "I bought the Raytop penthouse. Do you want to go stay there for a while?" and left, Momo (Hyeon Ran-hee's brain), who had seen him off listlessly, suddenly changed completely and exclaimed with a big laugh, "A 18-billion-won Raytop penthouse? Wow," exposing hidden ambition and sending chills.
While Momo (Hyeon Ran-hee's brain) showed curiosity by searching Ha Yong-jung on her phone, Geum Ba-ra arrived, and Momo (Hyeon Ran-hee's brain) surprised Geum Ba-ra by asking, seemingly lacking motivation, "Please run a retirement article about me." Furthermore, when Geum Ba-ra called Ha Yong-jung "oppa," Momo pressed, "Oppa? Are you dating?" and when Geum Ba-ra said, "Didn't we talk about it before? Don't you remember?" Momo deflected, "I've become a complete fool." However, Geum Ba-ra's puzzled look and the retirement article published under Geum Ba-ra's name caused a stir, foreshadowing further upheaval to come.
Meanwhile, Paul Kim (played by Gee Young-san) was pained to read the text Hyeon Ran-hee left before she died: "Tell her I'm very sorry." At that moment Momo (Hyeon Ran-hee's brain) visited Paul Kim and told him, "Before my mother died I heard that my consolation sister had been with her. The two of you were in high school... he was my sister's father, and he was my mother's boyfriend," revealing that she knew about their relationship. She then conveyed Hyeon Ran-hee's sincere wish to find her biological daughter through Momo (Hyeon Ran-hee's brain)'s words: "If you find her, she's my only blood relative. Please tell me." In particular, at the end of the episode, after Momo (Hyeon Ran-hee's brain) left, Paul Kim received news from an inquiry agency saying, "It seems we found your daughter," sparking attention on who Hyeon Ran-hee and Paul Kim's biological daughter might be.
